wait list letters

I was just wondering if someone could tell me what the waitlist letter looked like as opposed to the rejection letter. Does one or do both contain a separate yellow piece of paper? What does the letter open with (e.g. Dear Candidate, We're sorry to inform you.....etc., etc.)?
