Must-sign form?

Hey, just wondering, does OMSAS require us to mail them a must-sign form by the 15th? and if so.. where can I find this form online, I can't seem to see anything about a must-sign form, but some other schools require it so I thought I'd check.

I haven't heard of this form for OMAS applications, but I'm curious to see what others say!

Hi there,

 

I didn't see such a form, even when I submitted. However, I assume, in lieu of it, there is a couple of pages of legalia, to which you have to agree, before submitting your application.
