Sending in a Copy of the Printable Submission Form

Hi everyone! I submitted my USask application last weekend but came across the FAQ page today. For the first question which gives the address for sending in outstanding items, there is a note in red that reads: ** Include a paper copy of the "Printable Submission Form" available after you have submitted your online application.**

 

So let me get this straight, we print off the 1 pager and mail it into the Admissions office? Do we have to send anything else.. I'm confused!

I'm assuming this doesn't have to be sent in before the Nov. 1 deadline?
