Sports-Etudes consideration for Med-P

Hi everyone,

 

I just had a question about the sports-etudes application that McGill requires. From the site they say:

 

- Applicants are recommended to have an R score at or above 34.0. Applicants with R scores below 32.0 are rarely considered.

- (*1) Students enrolled in a Sport-études program may be exempted from these conditions. Such applicants must submit a special form (see Submitting an Application) which describes in detail all relevant sport activities. A sub-committee will determine the eligibility of the student.

 

I can't seem to locate this form anywhere on their site. Is there anyone who has successfully done it or can direct me to it? I've seriously looked everywhere for it.
