Hi everybody,

Im applying for the UBC MPT program and was having trouble filling out the initial part the referee confidential form.

Specifically what I should fill for the blanks for the following questions:

... who is applying for admission to study for the degree of __________ ( I would put Masters of Physical Therapy)

...in the Graduate Program of ________ (Physical Therapy??)

...Field of Study: __________ (Department of Physical Therapy or Faculty of Medicine)

 

I attached the actual form to see what it looks like (its the very first part at the top of the 1st page)
