For example, if I took a course in the summer after my first year (before my second year), would that grade be added to my first year GPA or would it be added to my second year GPA? Each school looks at grades in its own way, so you should refer to those school's guidelines/FAQs for information. Mac, for example, doesn't care what year you took anything in, so it doesn't matter if it's part of your first year GPA or second year GPA. It's all just your GPA.
