Fresh fry
I was under the impression that general IM is a subspecialty and you need it to practice. No matter what its a 3 + 1, unless I'm mistaken.

Fresh fry
I've heard of people with two subspecialties, not sure how they do it. IM has to gobthrough CaRMS for their subspecialty match so I imagine you could check the CaRMs site for specifics.
A-Stark
All IM training is at least 4 years. Generally people enter subspecialties after 3 years (applying in the summer/fall of R3), though Critical Care is often done after R4. Not all first iteration spots are available if applying as an R4, though.
