What kind of information is included in the MSPR?

Is the MSPR exclusively comments on clerkship rotations? Or does it include comments during years 1 and 2 (for example student performance during problem based learning sessions and clinical training sessions in year 1 and 2)?

No. The only comments we had on the MSPR were a maximum of 3 comments from electives and a maximum of 6 comments from core clerkship rotations. You could choose to include less. The comments were only taken from the end of rotation evaluations (although to be fair for some rotations they would just collate your daily evaluations as into a big wall of text comment on your final evaluation).
