Some do strictly outpatient consulting, and have overhead comparable to a family physician. Others do strictly inpatient work, and have minimal overhead. There are even a few that do only call shifts and nothing else, which can be lucrative and has no overhead. Most do a combination of in and outpatient work, however. For overhead, it depends on the contract they have at the hospital where they have privileges. If they have a clinic/office outside the hospital, they will have to pay those expenses themselves. If the clinic/office is in the hospital, they may have to pay the expenses, the hospital may pay them, or they may have to pay a fixed amount or percentage of the expenses, depending on their agreement.
