Questions regarding to writing the MCAT

Hi folks, 

A couple of newbie questions:

a. how many times are you allowed to rewrite the MCAT? Is there a lifetime limit? Is there a period limit (i.e. 5 times in a 2-year period)

b. when can I write the MCAT? I am planning to apply to all Ontario med schools, so therefore I must file my application(s) in the fall. Must I write the MCAT prior to applying or, as is the case with the LSAT, I can apply  and THEN take the MCAT?

c. Is it still the case that the MCAT is a cutoff criteria (considering that I am planning the apply only in Ontario). Btw I have a high cGPA, between 3.98 and 3.99 

d. any prep material recommendations/strategy would be greatly appreciated.
