On the referee info form, if I am filling it out for a Toronto doctor... should I just put Sunnybrook under organization name? Would it would be fine to also put U of T for insitution, in addition to Sunnybrook, if he is an ass. prof at UofT (like most Toronto hospital docs are)? Or are they just looking for one in the other?

I doubt that would be a problem :)
